Opioid Conversion Tables: How Accurate Are They?

چکیده

There are a number of reasons why physicians might want to switch from one opioid to another. Rotating opioids can reduce the risk of developing tolerance to the analgesic effects of any single drug and of developing hyperalgesia (where the extended use of an opioid can actually reduce the pain threshold, resulting in a worsening of the pain).2 Thus, changing to another opioid can be quite beneficial.

Opioid equianalgesic tables: are they all equally dangerous?

Pain is one of the most common symptoms in cancer patients. Opioids are widely prescribed for this and other purposes. Properly used, they are safe, but they have serious and potentially lethal side effects. Successful use of opioids to manage cancer pain requires adequate knowledge about opioid pharmacology and equianalgesia for the purpose of both drug rotation and route conversion. The aim o...

Review and critique of opioid rotation practices and associated risks of toxicity.

OBJECTIVES A dramatic increase in unintentional deaths from opioids has occurred over the past decade with strong inference that many of these deaths may be resulting from prescriber's error. Recent evidence suggests that the use of dose conversion ratios published in equianalgesic tables may lead to fatal or near-fatal opioid overdoses. The objective of this review was to determine whether the...

Opioid rotation in cancer patients: pros and cons.

Opioid rotation involves changing from one opioid to another using correct equianalgesic conversion techniques to achieve better analgesia and/or fewer side effects. The strategy appears to work because of significant interindividual variations in response to both analgesic activity and toxicity. Although there are many retrospective studies, few prospective controlled trials of opioid rotation...

Opioid agonist doses for oxycodone and morphine dependence: Findings from a retrospective case series.

INTRODUCTION Use of opioid agonist treatments for prescription opioid (PO) dependence is rapidly increasing. Current guidelines are based on research with heroin users. This study aimed to examine methadone and buprenorphine dose requirements for PO-dependent people. DESIGN AND METHODS A retrospective case series of PO-dependent patients entering methadone and buprenorphine treatment. Daily o...

Sampling Rate Conversion in the Discrete Linear Canonical Transform Domain

Sampling rate conversion (SRC) is one of important issues in modern sampling theory. It can be realized by up-sampling, filtering, and down-sampling operations, which need large complexity. Although some efficient algorithms have been presented to do the sampling rate conversion, they all need to compute the N-point original signal to obtain the up-sampling or the down-sampling signal in the tim...
